The Case Manager is responsible for establishing and maintaining positive working relationships and partnerships with outside agencies and care coordinators. The Case Manager also needs to communicate effectively in both verbal and written form within Focus to ensure all consumers receive outlined services. The Case Manager maintains on-going consumer contact with quarterly face-to-face visits to ensure familiarity with consumer’s individual needs and strengths, provides annual updates regarding progress on goals and writes new/revised goals for the annual plan of care and amendment plans of care.
